It's easy to Send a Smile by completing our online form using your credit card or by uploading your order and we'll call you for payment over the phone. Questions? Please email Donna Walker or call 570.674.3351.
    1. Download and complete the Send a Smile order form by clicking here. 

    2. Once you are happy with your order, save the PDF to your desktop and return to this page to submit your order.  

    ​3. Submit your order by clicking on Choose File (below) and upload the saved PDF from your desktop.

    4. Once the file uploads, click the submit button. A confirmation message will appear when your order is successfully submitted.
